St. John Paul II Award Nomination Form

To submit a nomination for the St. John Paul II Award, please complete the form below. All information provided will be kept confidential. Nominations are valid for one year at a time. If you do not have complete information of the nominee(s), please submit the form with what you have. The CCF staff will help you fill out the rest of the form.

As part of this nomination, please submit no less than one and no more than three pages of additional support for your nominee. This can include letters of recommendation, volunteer work, board service, fundraising experience, resumé, and parish-based ministry. Please include approximate number of years served in each. Significant volunteer service to Catholic institutions in our archdiocese	Practicing lay Roman Catholic or Deacon residing in the Archdiocese of New Orleans
High moral character	Not a candidate or currently holding public office
Exemplary personal values demonstrated	Not a current Catholic Community Foundation board member
